ABSTRACT

OBJECTIVE: To observe the therapeutic effect of electroacupuncture (EA) of "Zusanli" (ST36) and "Ashi"-point on the healthy side (opposing needling) on muscular injury and expression of myogenin (myoG) and fast myosin skeletal heavy chain (Fast MyHC) proteins in the gastrocnemius muscle (GM) tissues in skeletal muscle contusion rats,so as to explore its mechanism underlying improvement of skeletal muscle injury. METHODS: A total of 54 male SD rats were divided into normal control (n = 6),model (n=24) and opposing needling (EA, n=24) groups. The latter two groups were further randomized into 3, 5, 7 and 14 d subgroups (n=6 per subgroup). The skeletal muscle contusion model of the hind-limb was established by using a self-made striking device. EA (1 Hz/3 Hz,1-2 mA) was applied to ST36 and "Ashi"-point on the uninjured side of the hind-limb for 15 min every time, once a day for 3, 5, 7 and 14 days, respectively. The injured GM was harvested on the 3rd, 5th, 7th and 14th day after muscular contusion. The morphological changes of the injured GM and the mean cross-sectional areas (CSAs) of the neonatal muscle cells were observed by microscope after H.E. staining. The immunoactivity of desmin protein (myogenic marker protein of myoblast cell) of GM was detected by immunofluorescence stain on the 7th day after injury, and the expression levels of myoG (on the 3rd and 5th day after injury) and fast MyHC protein of GM tissues (on the 7thand 14th day after injury) were detected by Western blot. RESULTS: H.E. staining of GS tissue showed fewer neuronal myocytes with disordered arrangement at different sizes, and appearance of some collagenous fibers among the mesenchyme on day 7 and 14 after muscular contusion, which was relatively milder in the EA group. In the EA group, the CSA values of the neonatal muscle cells were significantly larger than those in the model group on the day 7th (P0.05). On the 3rd and 5th day after muscular contusion, the expression level of myoG protein was significantly up-regulated in the model group compared with the normal control group (P<0.001), and significantly up-regulated in the EA group than that in the model group (P<0.001). On the 7th and14th day after contusion, the expression level of fast MyHC protein was significantly down-regulated in the model group relevant to the normal control group (P<0.001), and markedly up-regulated in the EA group relevant to the model group (P<0.01).. CONCLUSION: EA of ST36 and "Ashi"-point on the contralateral limb can up-regulate the expression of myoG and fast MyHC proteins of GM in acute skeletal muscle contusion rats, which may contribute to its effect in promoting the repair of skeletal muscle injury.

ABSTRACT

Opposing needling is an acupoint selection method of acupuncture recorded in ('). And the first record of dragon-tiger fighting needling is found in the (), it is a compound reinforcing and reducing manipulation of tonification-purgation method. Both of them are widely used in the treatment of pain syndrome, which are mainly for nerve system and musculoskeletal diseases and seldom for gynecological diseases. By analyzing the pathogenic characteristics of chronic pelvic inflammation, the clinical application of opposing needling combined with dragon-tiger fighting needling for chronic pelvic inflammation is expounded based on the theory of treating different diseases with the same treatment in TCM.

ABSTRACT

It was to introduce professor 's experience in the treatment of oculomotor nerve palsy with acupuncture based on the interactions of twelve muscle regions and opposing needling theory. In the treatment, the TCM syndrome differentiation is taken as the general principle, the theory of the interactions of twelve muscle regions and opposing needling as the specific feature. The contrary needling is used at Hanyan (GB 4) and Juliao (ST 3) on the healthy side. The meridian-collateral and the exterior-interior differentiations are adopted to identify the syndromes. At the early stage of the disease, the treating principle focuses on expelling wind, eliminating cold and promoting the circulations of meridians and collaterals. The shallow needling, less acupoints and gentle stimulation are required. At the middle and late stage, the treating principle concentrates on strengthening the antipathogenic and harmonizing and blood. The deep needling, the multiple acupoints and the strong stimulation are applied. Additionally, the four-needling technique at Fengchi (GB 20), the multiple shallow needling at the eyelid margin, the opposing needling and the quantity of needling sensation are used in combination, which were developed by professor . With those comprehensivemethods, the symptoms of oculomotor nerve palsy are relieved and the significant clinical therapeutic effects achieved.

ABSTRACT

Objective To investigate the clinical efficacy of Dong’s extra point opposing needling in treating facial paralysis. Methods Ninety patients with facial paralysis were randomly allocated to treatment and control groups, 45 cases each. In addition to the basic medicine treatment, the treatment group received Dong’s extra point opposing needling and the control group, conventional acupuncture. Facial nerve function was scored using the House-Brackmann (H-B) Facial Nerve Grading System in the two groups of patients before and after treatment. The clinical therapeutic effects were compared between the three groups.Results There were statistically significant post-treatment differences in the H-B scores of various time points between the treatment and control groups (P<0.05). The total efficacy rate and the cure rate were 100.0% and 68.9%, respectively, in the treatment group and 100.0% and 48.9%, respectively, in the control group. There was a statistically significant difference in the cure rate between the two groups (P<0.05). The time needed for cure was (20.15±3.30) days in the treatment group and (28.41±4.92) days in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Dong’s extra point opposing needling plus medication is an effective way to treat facial paralysis.

ABSTRACT

ObjectiveTo observe the efficacy of contralateral needling plus point-toward-point method in treatinghemifacial spasm, and to seek the optimal treatment protocol by combining clinical data.MethodNinety-four patients with hemifacial spasm were randomized into a treatment group and a control group, 47 cases in each group. The control group was by acupuncture at the affected side, while the treatment group was by contralateral needling plus point-toward-point method. The therapeutic efficacy was analyzed by comparing the changes of symptoms and the recovery of the spastic facial tissues.ResultThe symptoms in the treatment group were effectively mitigated, and the effective rate was 97.9%, significantly higher than 83.0% in the control group (P<0.05).ConclusionContralateral needling plus point-toward-point method is significantly effective in treating hemifacial spasm, and it benefits the recovery of the facial muscle function.

@# Firstly effect of opposing needling was underlying acupoints of uninjured side facilitate injured side, similar as the nature of association reaction. That hinted association reaction may be the mechanism of opposing needling. Then opposing needling about its clinical application indication, therapeutic effect, stimulus intensity and strategy was discussed and interpreted on the basis of apprehension association reaction conception, essential characteristic and its role in hemiplegia rehabilitation.

@#ObjectiveTo observe the clinical effect of opposing needling combined movement method on upper limb spasm of hemiplegic patients.Methods63 hemiplegic patients with upper limb spasm were randomly divided into 2 groups. The treating group received opposing needling combined movement method. The control group received only movement method.ResultsThe rate of efficiency of the treating group was 90.6%, and that of the control group was 64.52%. There was a significantly difference between the two groups (P<0.05).ConclusionThe effect of opposing needling combined movement method on upper limbs spasm of hemiplegia is better than that of single movement method.

@#ObjectiveTo explore an effective rehabilitation method to treat hemiplegic patients with stroke by acupuncture.Methods62 hemiplegic patients were treated by Opposing needling and Non-opposing needling twice a day in alternate way.The motor function of patients was assessed with Brunnstrom Assessment and activities daily living(ADLs) were assessed with Barthel Index revised.ResultsAt 3 months after the treatment,The rate of motor function improved above Ⅳ-Ⅳ level accounted for 62.90%,that of ADL improved 60 points accounted for 56.45%.There were significant difference in Brunnstrom level among pre-treatment and 1,2 months after the treatment(P<0.01),but no significant difference between 2 and 3 months (P>0.05).ConclusionsOpposing needling and Non-opposing needling used twice a day in alternate way can promote the recovery of motor function and ADL of hemiplegic patients with stroke,especially early (less than 2 months) recovery.
